Audit Case Study: Documentation and Coding Reminders

AADSM recently reviewed an audit case involving oral appliance therapy (OAT) claims in which documentation and billing practices were closely evaluated. Proper documentation is a key component to record keeping. Here are some key points to remember:

Clinical Documentation:  
Ensure that patient records clearly document the services performed by the treating dentist. Documentation should reflect the dentist’s direct involvement in evaluation, appliance delivery, and follow-up care. Depending on the policy and state regulations, delegation of certain tasks may be appropriate, but the treating provider MUST properly document and sign off on the services they provide.  
 
Appliance Coding Accuracy 
It is critical to verify correct HCPCS coding when billing for oral appliances. Providers should confirm whether the appliance billed aligns with E0486 or K1027 by referencing the Pricing, Data Analysis, and Coding (PDAC) database.
 
Payer-Specific Policy Compliance:  
Review and adhere to plan-specific coverage criteria, medical necessity requirements, documentation standards, and billing guidelines before submitting claims. Coverage policies frequently change, so it is important to stay up-to-date on any changes.
